Our Direct Support Professionals will support your participation in activities of your choice throughout the community. Go shopping, banking, use library services, volunteer, and more.
Supported Living Services
Our Supported Living services include habilitation, personal care, nursing, and other residential supports. We will help you gain, maintain, and improve skills related to activities of daily living. Our services are offered in a home serving one to three people. Transportation can be optional.

Ihoma In-Home Supports provides periodic services to enhance your quality of life by encouraging you to develop independence at home and in the community. Whether you live on your own or with family or friends, we can help you gain, maintain, and improve the skills you need to be successful.

How do In-Home Supports help?
Our In-Home Supports staff will help you learn and maintain daily living skills, manage behavior issues, and attend social outings in the community. We provide a combination of training, hands-on care, and support to:

learn, master, and maintain daily living and independent living skills

use community resources to increase your participation and integration

enhance your ability to manage your finances and personal affairs
-
build awareness of health, nutrition, physical status, and safety precautions

coordinate or manage tasks in your Health Management Care Plan

perform personal care, household, and homemaking tasks

develop skills necessary to reduce or prevent behavior issues

coordinate transportation to and from events and activities in the community

Residential Habilitation Services

What are Residential Habilitation Services?

Residential Habilitation services are supports provided in a shared home setting of four to six people. These services offer ongoing training, support, and supervision.
At Ihoma Healthcare, we provide Residential Habilitation services that are tailored to you. Our staff will help you learn, maintain, and improve skills related to self-care, activities of daily living, adaptive functioning, and other areas that enhance your independence.
You will receive hands-on support, habilitation, and other services in line with your Individual Support Plan (ISP) and Plan of Care. A few of the benefits can potentially include:

Respite Services
What are Respite services?
Do you care for someone enrolled in the Waiver? There may be times when you need extra support - whether it's an emergency, an appointment, or you just need some time for yourself. Ihoma Healthcare's respite services can provide you with short-term, temporary relief. Our Respite services will ensure that your loved one continues to receive services while you are away. This includes access to community activities as described in their Individual Support Plan (ISP)/Plan of Care.

Daily respite care (in an Ihoma Healthcare home) or hourly respite care (in the home of the person supported)

Coordination and provision of transportation and access to and from community activities
